
Members of our community diabetes team who offer diabetic patients support, advice and treatment in their local GP surgery.
There are nearly 7,000 people in our practices diagnosed with Type 2 diabetes. We have launched a Community Diabetes Service so they can be treated in the familiar surroundings of their own GP surgery, by clinical staff they know, rather than having to go to hospital.
Our specialist team run community support clinics to help diabetic patients better understand and manage their own condition and live a more independent life.
The consultant led team includes specialist community nurses and dieticians. They offer patients support with diet, lifestyle, new therapies and getting started on insulin.
They also provide training for practice nurses so they can offer ongoing care for patients who currently have to attend hospital outpatient clinics.
